What Is Sciatica?

Sciatica is pain that radiates along the sciatic nerve, from the lower back through the hip and down the leg. It is usually caused by nerve compression from a herniated disc, spinal stenosis, or tight surrounding muscles.

Because sciatica stems from nerve irritation rather than the leg itself, treatment focuses on relieving pressure at the source in the lower spine.

How We Evaluate Sciatica

Your first visit starts with a full history and a physical exam, including orthopedic and neurological tests to help identify where the nerve irritation is coming from and how it's behaving. Most cases of sciatica can be evaluated and treated without imaging.

If your symptoms, exam findings, or response to care suggest something that needs a closer look, such as significant or progressive weakness, or symptoms that don't improve with conservative care, Dr. Demsky will recommend imaging or refer you to the appropriate physician or specialist.

Seek immediate medical attention (call 911 or go to the ER) for sudden loss of bladder or bowel control, progressive weakness or numbness in both legs, or sciatica following a significant trauma — these can indicate a medical emergency that chiropractic care is not appropriate for.

Your first visit runs about 45 minutes and includes the exam and your first treatment. Follow-up visits are typically 15–25 minutes depending on what's included.

Do I need a referral or imaging first?

No referral is needed, and most sciatica cases can be evaluated without imaging first. If imaging or a specialist is warranted, Dr. Demsky will let you know and help coordinate next steps.
